Ordering Information

Please complete an order form and/or have the desired product name, part number, and quantity readily available.

Payment

Orders will be processed upon receipt of a valid purchase order. Terms are net 30 days, and the customer will be invoiced for product upon date of shipment. All accounts over 30 days are subject to a 1.5% interest charge on unpaid balance. Acceptable means of payment are cash, check or credit card.

Warranty

All Top Shelf Orthopedics products have a limited warranty from the date of purchase against defects in materials and workmanship. The warranty by product family is as follows:

Shipping

Unless specified otherwise, Top Shelf Manufacturing ships using UPS ground. Shipping charges are the responsibility of the customer. Orders received before 2:00 pm Pacific Standard time will be shipped within 24 hours. Top Shelf will drop product shipment upon request.

Return Policy

• All returns are subject to a restocking charge at the discretion of Top Shelf Orthopedics.
• All returns must be preauthorized by Customer Care and be returned with a RETURN AUTHORIZATION number.
• Products must be returned in original packaging.
